It’s 2024. The year that almost ALL Physical Therapists are beginning to realize the writing is on the wall for being a PT stuck inside of the sinking insurance system.
The once reliable and profitable “SS Insurance” is a ship that is well untruly sinking. It’s going down faster than the Titanic, and the APTA is powerless to stop it.
If I were asked to describe the current situation if you’re insurance-based PT I could sum it up in two words:
“OH CRAP!”
And I wish I was joking, but I am not.
There’s literally been an “OH CRAP!” moment – a moment of reconning if you like – for 10,000’s of insurance-based PT clinic owners since the LATEST round of Medicare cuts (and other PPO’s) were announced.
The latest cuts are set to make running a PROFITABLE PT clinic almost impossible.
And the thing is, most sane people know it.

She’s one of many caught in the rat-hole system that is insurance headaches, high volume, low pay, and needing to wait on hold for hours to discuss payment only to be denied.
It’s bad enough that she isn’t making any profit – but what makes it even worse is that she thinks that there isn’t a single person in her area that will go self-pay (…the reality is this isn’t true – she just doesn’t know how to market to them!)
But the problems don’t stop with Insurance based PTs…
If you’re a CASH based PT, and you’ve already exited the crap-tip system, or decide against it completely, first of all WELL DONE to you. It takes courage to make such a bold move.
However, just because you made the decision, you won’t need me to tell you how increasingly difficult it is to SCALE a cash PT business.
The constant negativity around healthcare, the rising costs associated with healthcare, and the lack of LOVE in general for people in the USA wanting to spend money on non-essential treatment like PT is making it very difficult for you to scale your practice.
Add into the fact that there’s now hundreds of “pop up” PT clinics appearing in every location possible – from your local YMCA to renting a small room in the fitness or Yoga centre down the street – the competition for patients is hotter than ever.
So, what do you do?
On the one hand you have a valuable skill that people NEED, but on the other hand, most people really don’t know all that much about PT unless they hear it from a doctor. And if they here it there first, they usually want to use insurance. It’s almost impossible to get a cash paying patient from a patient referred by a doctor.

It’s a SCARY time right now, the economy is turbulent, politicians have lost their mind and the media is filling people with FEAR on a bigger scale than ever before. That can cause mostly rational people to do irrational things.
However, this event is not to be missed. The fact that the economy is volatile is THE reason to come and let’s face it, politicians have always been a sham, and the media’s job is to install fear. It’s how they get better ratings.

I also know that you’re probably UNDERCHRAGING right now and I know that bugs the life out of you.
I will bet you’re charging at least 25-50% less than my other clients which when fixed could add up to $100K or more if you come and implement what I tell you…
